Form 1 Information
Beginning January 1, 2024, all Supervisor Statement of Financial Interest Forms will be filed out online and submitted to the Florida Commission on Ethics instead of your local Supervisor of Elections' office. You should receive an email from the Florida Commission on Ethics when it comes time to submit your Form 1s. A link to the Florida Commission on Ethics website with links to the Forms and instructions on how to fill them out can also be found here: Forms - Ethics. What is expected of a CDD Supervisor?
The CDD Board meets monthly and Supervisors are expected to attend to vote on matters concerning the operation and maintenance of the District's infrastructure (including, but not limited to, the District's stormwater system, common area landscaping, roads, and amenity facilities), in accordance with the District's budget, and the District's insurance and Bond obligations. The organizing of community social activities and events, and matters concerning private property or property outside the District's boundaries do not fall within the District's purview. District staff implement the Board's directions and policies in accordance with Florida Statutes, and the District's Rules of Procedure and ensure vendors operate in accordance with the District's policies and agreements.
All Supervisors are required to operate under Florida's Sunshine Laws and abide by Florida's Public Officers and Employees Code of Ethics, and are also required to submit an annual financial disclosure (Form 1) with the Florida Commission on Ethics and complete four hours of ethics training per calendar year. District Counsel will explain these requirements in more detail to appointed/elected Supervisors.

Below is a list of the Heritage Harbor CDD Board of Supervisors with their assigned positions and terms, along with information from Hillsborough County Supervisor of Elections regarding running for office.

Candidates may file and begin running for office at any time, by completing and submitting the DS-DE 84 (PDF) and either an Affidavit of Intention (PDF) or a DS-DE 9 (PDF). The DS-DE 9 must be submitted if the candidate intends to spend money campaigning or collect campaign contributions; otherwise the Affidavit of Intention should be filed.
The candidate pre-qualifying period begins May 25. Hillsborough County Supervisor of Elections will provide qualifying documents upon request via email to [email protected] about a month prior to the pre-qualifying; those documents will also be posted on the Hillsborough County Supervisor of Elections website. All qualifying documents and fees must be received no later than noon, June 12. Please note that filing and qualifying documents can be mailed or dropped off at any Hillsborough County elections offices. They cannot be accepted electronically.
